Rates Are Negotiable

Private Pay Mileage Reimbursement Rate: .49 Cents per
mile. These are for any incidental
transportation such as:

• Appointments
• Errands
• Visiting friends or relatives
• To and from events and activities if and when the employees vehicles are being used.

We do have a monthly private pay policy and we
accept payment in cash, checks or money
orders. The 1st of each month an invoice and
copy of time-sheet will be send to client for
hours worked by Independent Choices
employee stating what is owed for previous
month. Payment will be due in full by the
10th of each month. Unless arrangements are
made with management. Rates are subject to
change but contracted clients would be
notified in writing. Holidays hours will be
billed at time in a half.
